Under OSHA 1910.333(a)1; there are only two instances in which an employee can work on live parts. A qualified person must complete and approve a work permit in these situations.
- When de-energizing would interrupt essential life support, emergency alarms, or ventilation systems.
- When the organization can demonstrate that de-energizing the system would introduce additional or increased hazards or infeasible due to equipment design or operational limitations.
To request a Hotwork Permit on a Job or Work Order using a mobile device:
- Click the down-arrow to expand the Actions menu.
- Click 'Request a Hotwork Permit'.
To request a Hotwork Permit on a Job or Work Order using a desktop PC:
- From the dashboard, click the drop-down Actions menu at the top of the screen.
- Click 'Request a Hotwork Permit'.
The form "Request Permit" is displayed.
- Complete all the fields on the form.
- Click the 'Submit' button.
- Contact your Foreman or Supervisor if you have any questions.

When finished, click 'Submit'.
Once submitted, the completed form is emailed to the address listed in Settings.
